CRB AND/OR DNA (EXHIBIT “B”)

By my signing this agreement I am hereby specifically acknowledging the following:

1. I have received and reviewed a copy of an outline of DNA’s/CRB’s rights and responsibilities pursuant to the terms of their mutual consent (settlement) agreement with the FTC (Exhibit “A”).

2. That pursuant to the technical terms of CRB’s/DNA’s consent (settlement) agreement with the FTC, I must affirmatively enter in to a new agreement with CRB/DNA if I desire for them to continue to provide services under a new agreement.

Therefore, by my signing this agreement, I am hereby specifically entering in to a new agreement with CRB and/or DNA. The terms of the new agreement adopt and incorporate herein the identical terms, conditions, rights, and obligations of my prior agreement with CRB and/or DNA.

In addition to the terms and conditions of this new agreement, I acknowledge that I have read and understand the following disclosures (the following disclosures apply to debt clients only).

1. “Fees and conditions apply. Ask for details.”
2. Consumers who purchase debt relief services will be able to satisfy their debts by paying substantially less than the full amount of those debts.
3. The total cost to purchase, receive, or use any or use any goods or services; 19% of the included debt plus $24.95 per month (already included in the previous contract).
4. That consumers’ balances will typically increase during this time period until settlements for all accounts are actually achieved.

I further acknowledge and represent that I am freely and voluntarily entering into this new service agreement, and that no one on behalf of CRB and/or DNA has initiated contact with me first after the date of the order. I, however, may have contacted CRB and/or DNA first without solicitation, requesting that CRB and/or DNA contact me to discuss my desire to enter in to a new agreement with CRB and/or DNA after the date of the entry of the order adopting the FTC agreement.
